HOW TO REQUEST A GRANT

  2. Request Grants
  1. Submit the “Application for Grant” form for the current year. We prefer it to be completed in english. (Application will be available during the month of july.)
  2. Submit the following documentation with the application:
    • Information about the members of the Board of Directors: Name, officer title and term, physical and electronic address, telephone, and place of work.
    • Non-profit certification issued by the Puerto Rico Department of State.
    • Current Tax Exemption Certification issued by the Treasury Department.
    • Current certificate of Good Standing status issued by the P.R. Department of State.
  3. Current operating permits and licenses including:
    • Single Permit
    • Occupancy Permit
    • Fire Department
    • Education Department
    • FamilyAffairs Department
    • Health Department
    • Puerto Rico Mental Health and Anti-Addiction Services Administration (ASSMCA)
  4. Most recent audited financial statements available (Within (6) months of the institution fiscal year closing. If audited financial statements are not available, please contact us before submitting your application)
  5. Detailed operational budget for the year in which donations are requested.
  6. Three quotes for the articles intended to be acquired with the donation.
  7. Puerto Rico Treasury Department Certification of Income Tax Return Filing for the last 5 years.
  8. Certification of No Outstanding Debt, ( if the entity has a payment plan, please provide evidence that the plan is up to date)
    • Puerto Rico Treasury Department
    • Labor Department
    • Internal Revenue Service (IRS)
    • Workmen’s Compensation
  9. Map indicating how to arrive to the entity.

  11. All applications should be submitted electronically or by mail. No applications will be accepted by fax or personally delivered at our office.
